Can you help us understand how GLP-1 medications work in simple terms?
"Imagine your body has a special lock (receptor) designed to receive a specific key (GLP-1). This key, GLP-1, is a natural gut hormone that helps regulate blood sugar and digestion. A GLP-1 agonist is like a copy of this key, fitting the same lock and mimicking the effects of the natural hormone.
In simpler terms, a GLP-1 agonist is a medication that acts like a natural hormone in your body to help manage blood sugar and curb appetite by lowering blood sugar levels, slowing stomach emptying, and increasing the feeling of fullness (satiety)."
What are the main problems that patients of GLP-1 face?
"While GLP-1 medications offer significant benefits, they are not without their pros, cons, and side effects. Common side effects include nausea, vomiting, and constipation, with some patients experiencing severe nausea.
One of the biggest issues that patients may not think about at first is how rapidly they may lose muscle. When you’re losing weight at a rapid pace, yes you are losing fat, but along with that your body is going to start to shed muscle quickly as well. This can lead to a host of problems, least among them being aesthetic expectations."
Is strength training something that GLP-1 patients should be focusing on?
"Rapid weight loss from GLP-1 medications can lead to a decrease in muscle mass, reduced bone density, and a lower resting metabolic rate, contributing to sarcopenia (gradual loss of muscle mass and strength). To combat this, it is crucial to increase protein intake and incorporate strength and resistance training while on this type of GLP-1 journey.
The goal for anyone on this medication, should be to improve body composition by shedding excess fat, particularly visceral fat, while increasing lean mass, including muscle and bone. Clinical trials are examining the effects of GLP-1 agonists on body weight, BMI, and body composition, underscoring the importance of protein intake and strength training."
What cautions would you give to GLP-1 patients as they embark on their journey?
"One of the most important considerations is making true lifestyle changes while on GLP-1 medications. The drugs will work as long as the side effects are tolerable, but without lasting lifestyle modifications, the weight is likely to return if old habits resume.
Achieving lasting results requires dedication and hard work. Most individuals, after experiencing significant weight loss, are motivated to avoid reverting to previous lifestyles. Utilizing GLP-1 medications as a tool for sustainable change involves incorporating healthy eating habits, regular physical activity, and ongoing support."
Do you have any advice for patients who want to use GLP-1 and strength training in tandem?
"Ensuring that you’re following a program focused on growing or retaining muscle is key. You most likely don’t want to do all cardio or HIIT based workouts since you’re already rapidly losing weight. Focusing on muscle-retention while you’re on the medication is incredibly important.
From a dietary standpoint, remember that your preferences may change and it may be difficult to eat the appropriate caloric intake, but you should really try to. Finally, the potential for loose or excessive skin post-weight loss, which can affect body image and identity is a definite possibility. These considerations highlight the need for a comprehensive discussion with healthcare providers and/or your Sesh+ Coach."
